A natural escapement looks nothing like a Swiss lever

Two escape wheels impulsing the balance directly, with no pallet fork mediating between them and the balance, is a visibly different architecture from the Swiss lever escapement in essentially every other watch. It cannot be approximated by decorating a conventional movement, and it is visible through the caseback. Where a watch is presented as carrying one, this settles the question.

Watch the tourbillon carriage actually rotate

A tourbillon carriage rotates, conventionally once a minute, carrying the balance and escapement with it. Cheap watches with a rotating disc in a dial aperture — and there are many — do not. The distinction is between a carriage carrying an oscillating balance around with it and a decorative element turning behind an opening, and a few seconds of observation separates them.
